This form tells us about your player. We will keep this information on file for one year before asking you to complete a new form. You may update this information at any time by completing a submitting new form.
Step Two: Participant Waiver

Anyone who will be participating in activities inside of the training area the Facility must have a signed waiver on file. Coaches too! Parents: please choose “Minor” to complete this waiver for your youth player. Waivers are valid for one year.
Step Three: Review Facility Rules & Safety Guidelines and Code of Conduct
In the waiver you acknowledge that you (or your youth player) will adhere to our Facility Rules & Safety Guidelines and Code of Conduct. These items are posted at the Facility and available for review at any time here:

Step Four: Enjoy Activities at Our Facility!
Your player registration is now complete! In the future when you book private lessons, camps or clinics, your information will already be on file – you will only need to checkout and you’ll be all set.
